Prepare For Your Appointment

Prescriptions and refills are typically handled during scheduled office visits, and enough medication will be prescribed to last until the patient’s next appointment. If you run out of medication, it may be a sign that it is time to schedule a follow-up visit. Patients should be seen annually for routine medication refills.
If your insurance plan requires a prior authorization, you are responsible for obtaining the Prior Authorization Form from your insurance company for your physician to complete. While our staff and physicians do their best to assist, insurance and drug coverage plans are contracts between the patient and their insurance provider.
If you need to change pharmacies or switch to mail-order service, Ohio law allows non-expired prescriptions to be transferred without interruption. However, many mail-order pharmacies require prescriptions to be rewritten. If this is needed before your next scheduled visit, you may:
1. Make an appointment for repeat refills
2. Wait until your next regular visit, or
3. Pay a fee for your physician to review your medical record and rewrite the prescription (to be picked up at the office)
Please note that lost prescriptions for controlled medications (such as pain medications or sleep aids) cannot be replaced.
We discourage calling in prescriptions, as it prevents proper evaluation of your condition and the appropriateness of the medication. We also do not accept refill requests directly from pharmacies. When pharmacies contact us, we ask that they have the patient reach out to us directly.

Late Cancellation and No-Show Policy

We understand that plans can change. If you need to cancel or reschedule your appointment, we're here to help.

To keep appointments available for all patients and reduce wait times, please contact us at least 24 hours in advance if you cannot make your scheduled visit.

What Counts as a Late Cancellation or No-Show?

Late Cancellation: Canceling or rescheduling your appointment less than 24 hours before the scheduled time.

No-Show: Missing your appointment without notifying us at least 24 hours in advance.

Fees for Missed Appointments

Appointments not canceled at least 24 hours ahead of time will incur a $25 fee.

Please note: This fee is not covered by insurance and will be billed directly to you.

How to Cancel or Reschedule

You may cancel or reschedule your appointment in any of the following ways:

  • MyChart: If you're enrolled, you can cancel or reschedule anytime through your account.
  • Call the office: If we're unable to answer, please leave a voicemail with your full name, date of birth, appointment date/time, and the reason for cancellation.

Helpful Reminder

Please arrive 15 minutes before your scheduled appointment to allow time for check-in and any necessary paperwork. This helps us stay on schedule and ensures your visit runs smoothly.
