Prepare For Your Appointment
Appointments & Referrals
Appointments can be made by calling (614) 322-9640. Telephone calls during regular office hours will be answered by the reception staff. Messages will be given to your individual physician’s medical assistant. All messages, including those requiring physician input, will be answered the same day. Same day appointments are available for acute illness visits.
For your safety and in the interest of obtaining the best medical diagnosis, we ask that you come into the office for any new symptoms. We believe treatment over the phone is not optimal medical care.
When You Visit
You will be greeted by our highly competent, personable office staff. You will be promptly escorted to a clean, state-of-the-art exam room. After seeing your physician, we have an on-site phlebotomist available for any necessary lab draws. Our office is conveniently located very near facilities for X-rays, CT Scans, ultrasounds, stress tests, echocardiograms, and MRIs. We will ask that all insurance and address information be updated at each visit and any co-pays or past due balances be paid at the time of the visit.
Prescription Policies and Information
We have an automated voice mail prompt for refill requests available 24 hours per day. Refills can be faxed directly to your local or mail order pharmacy within 48 hours. The following information is required to complete your refill request: Your name (with spelling) and date of birth, your daytime phone number in case of any questions, your doctor’s name, your pharmacy’s name, street address, and phone number, name of the requested drug with spelling, strength and frequency taken, number of days’ supply needed (30 or 90 day supply with refills).
Late Cancellation and No-Show Policy
We understand that plans can change. If you need to cancel or reschedule your appointment, we're here to help.
To keep appointments available for all patients and reduce wait times, please contact us at least 24 hours in advance if you cannot make your scheduled visit.
What Counts as a Late Cancellation or No-Show?
Late Cancellation: Canceling or rescheduling your appointment less than 24 hours before the scheduled time.
No-Show: Missing your appointment without notifying us at least 24 hours in advance.
Fees for Missed Appointments
Appointments not canceled at least 24 hours ahead of time will incur a $25 fee.
Please note: This fee is not covered by insurance and will be billed directly to you.
How to Cancel or Reschedule
You may cancel or reschedule your appointment in any of the following ways:
- MyChart: If you're enrolled, you can cancel or reschedule anytime through your account.
- Call the office: If we're unable to answer, please leave a voicemail with your full name, date of birth, appointment date/time, and the reason for cancellation.
Helpful Reminder
Please arrive 15 minutes before your scheduled appointment to allow time for check-in and any necessary paperwork. This helps us stay on schedule and ensures your visit runs smoothly.
