If you are able to print off the new patient forms shown on this website, we ask that you fill these our completely and bring them with you or, come in 15 minutes before your appointment time to fill them out while in the office. Also, please bring all medication that you are currently taking so we can go over them with you.Prescription Policies and InformationPrescription refills will be handled at office visits, and enough medicine will be prescribed to last at least until the patient’s next appointment. We discourage the practice of calling in prescriptions as it does not allow the patient or the doctor to re-evaluate the appropriateness of the prescription, and follow the condition for which the medication has been prescribed. We do not accept calls from pharmacies to refill medications. When pharmacies call us, we request contact directly with the patient. Since we expect to fill them at the office visit, running out may be a signal that the patient is due in the office and needs to be scheduled. Our policy for prescription refills for patients changing pharmacies or changing to mail order: If a patient decides to change pharmacies, the law in the State of Ohio permits non-expired prescriptions to be transferred to the new pharmacy without charge or interruption. In the case of most mail order pharmacies, they disregard this provision and require prescriptions to be re-written. In the event that this is preferred by the patient BEFORE their next routine visit, the patient has a couple of choices:Make an appointment to see the doctor for repeat refillsWait for their next regular visit to change pharmaciesPay a fee for the patient’s physician to review their medical records and re-write the prescription – which will then need to be picked up at the office. Late Cancellation and No-Show Policy

We understand that plans can change. If you need to cancel or reschedule your appointment, we're here to help.

To keep appointments available for all patients and reduce wait times, please contact us at least 24 hours in advance if you cannot make your scheduled visit.

What Counts as a Late Cancellation or No-Show?

Late Cancellation: Canceling or rescheduling your appointment less than 24 hours before the scheduled time.

No-Show: Missing your appointment without notifying us at least 24 hours in advance.

Fees for Missed Appointments

Appointments not canceled at least 24 hours ahead of time will incur a $25 fee.

Please note: This fee is not covered by insurance and will be billed directly to you.

How to Cancel or Reschedule

You may cancel or reschedule your appointment in any of the following ways:

  • MyChart: If you're enrolled, you can cancel or reschedule anytime through your account.
  • Call the office: If we're unable to answer, please leave a voicemail with your full name, date of birth, appointment date/time, and the reason for cancellation.

Helpful Reminder

Please arrive 15 minutes before your scheduled appointment to allow time for check-in and any necessary paperwork. This helps us stay on schedule and ensures your visit runs smoothly.
